Race-conscious serious illness communication: An interpersonal tool to dismantle racism in practice and research

Background
Racism significantly contributes to inequitable care quality and outcomes for people of color with serious illness, their families, and their communities. Clinicians use serious illness communication (SIC) to foster trust, elicit patients’ needs and values, and deliver goal-concordant services. Current SIC tools do not actively guide users to incorporate patients’ experiences with racism into care.
Objectives1) To explicitly address racism during SIC in the context of the patient’s lived experience and 2) to provide race-conscious SIC recommendations for clinicians and researchers.
MethodsApplying the conceptual elements of Public Health Critical Race Praxis to SIC practice and research through reflection on inclusive SIC approaches and a composite case.
ResultsPatients’ historical and ongoing narratives of racism must be intentionally welcomed in physically and psychologically safe environments by leveraging empathic communication opportunities, forging antiracist palliative care practices, removing interpersonal barriers to promote transparent patient–clinician relationships, and strengthening organizational commitments to strategically dismantle racism. Race-conscious SIC communication strategies, skills, and examples of talking points are provided.
DiscussionRace-conscious SIC practices may assist to acknowledge racial dynamics within the patient–clinician encounter. Furthermore, race-conscious SIC may help to mitigate implicit and explicit bias in clinical practices and the exclusionary research cultures that guide them.
Travel Sickness and Meat Quality in Pigs

An experiment was conducted to investigate the incidence of travel sickness in pigs, specific hormone concentrations at exsanguination and subsequent meat quality. Fifty, 80kg slaughter pigs were transported on a lorry for 4.5h. During the journey, behavioural observations of the individually marked pigs were made by scanning every 8min to establish whether the pigs exhibited certain symptoms of travel sickness (foaming at the mouth and chomping) and incidences of retching and vomiting were noted as they occurred. Upon arrival at the slaughterhouse, pigs were unloaded, slaughtered immediately and a blood sample was taken at exsanguination for analysis of plasma Cortisol, beta-endorphin and lysine vasopressin concentrations. On the day following slaughter, the chilled carcase of each pig was assessed for meat quality (using pH, Fibre Optic Probe, and Tecpro Pork Quality Meter measurements) in the longissimus dorsi, semimembranosus and adductor muscles to determine the incidence of PSE (pale, soft, exudative) or DFD (dark, firm, dry) meat quality. Twenty-six per cent of the pigs (a total of 13 individuals) vomited or retched during the journey. There was no relationship between the incidence of travel sickness and either the concentrations of the hormones analysed at exsanguination or subsequent meat quality. Correlations revealed no significant relationship between concentrations of the hormones and meat quality measurements.

Significant new opportunities for astrophysics and cosmology have been identified at low radio frequencies. The Murchison Widefield Array is the first telescope in the southern hemisphere designed specifically to explore the low-frequency astronomical sky between 80 and 300 MHz with arcminute angular resolution and high survey efficiency. The telescope will enable new advances along four key science themes, including searching for redshifted 21-cm emission from the EoR in the early Universe; Galactic and extragalactic all-sky southern hemisphere surveys; time-domain astrophysics; and solar, heliospheric, and ionospheric science and space weather. The Murchison Widefield Array is located in Western Australia at the site of the planned Square Kilometre Array (SKA) low-band telescope and is the only low-frequency SKA precursor facility. In this paper, we review the performance properties of the Murchison Widefield Array and describe its primary scientific objectives.

EMU is a wide-field radio continuum survey planned for the new Australian Square Kilometre Array Pathfinder (ASKAP) telescope. The primary goal of EMU is to make a deep (rms ∼ 10 μJy/beam) radio continuum survey of the entire Southern sky at 1.3 GHz, extending as far North as +30° declination, with a resolution of 10 arcsec. EMU is expected to detect and catalogue about 70 million galaxies, including typical star-forming galaxies up to z ∼ 1, powerful starbursts to even greater redshifts, and active galactic nuclei to the edge of the visible Universe. It will undoubtedly discover new classes of object. This paper defines the science goals and parameters of the survey, and describes the development of techniques necessary to maximise the science return from EMU.

We define four distinct thermohydrochemical environments for drip shield and waste package corrosion in the potential nuclear waste repository, referred to here as the Dry, Seepage + Evaporation, Seepage + Condensation + Evaporation, and the Seepage + Condensation environments. These environments are bounded by temperature and relative humidity conditions at drift wall and drip shield/waste package surfaces judged most likely to initiate fundamental changes in the quantity and/or chemistry of in-drift waters. The duration in which different environments might exist is evaluated by comparing simulated, time-dependent temperature and relative humidity curves for three different locations within repository drift 25. In-drift conditions and processes postulated to cause drip shield/waste package corrosion are evaluated within the context of the thermohydrochemical environments by various means, including analytical calculations and geochemical simulations. Of the four environments considered here, the Seepage + Evaporation environment presents the most significant potential for aqueous corrosion of drip shield and waste package materials, and may persist for approximately 500 years in center drift locations. The likelihood for corrosion in other thermohydrochemical environments is significantly lower, but may increase with the acquisition of new data or the demonstration of extenuating circumstances.

Radiation damage experiments are being performed with pyrochlore and zirconolite in support of the disposition of surplus weapons-ready Pu. Pyrochlore becomes amorphous in approximately 1 year from the alpha recoil damage of ∼ 1018 alphas/g from the decay of 238Pu. The dissolution rate of 238Pu-bearing ceramics increases with increasing radiation damage as measured in a 3 d MCC-1 test at 90°C. Over the same period, zirconolite retains substantial crystallinity albeit with broadened diffraction peaks. The dissolution rate also increases with increasing radiation damage.

Forsling et al. (1984) have shown that exposure to vibration and noise leads to raised concentrations of plasma lysine vasopressin (LVP) in pigs. Pigs can exhibit symptoms of travel sickness during road transport even when, following commercial practice, they are not fed before transportation. These symptoms appear to be associated with elevated concentrations of plasma LVP (Bradshaw et al. in press). We wished to establish whether concentrations of plasma LVP at exsanguination may reveal which pigs had been travel sick during the journey to slaughter and whether those pigs exhibited subsequent poor meat quality.
Fifty 90 kg slaughter pigs were transported on a lorry (25 each day for two days, food withdrawn the previous evening at 1700) for five hours (0.49 m2 per pig). RHB travelled in the main body of the vehicle scanning the individually marked pigs every 8 min for incidences of standing, lying and symptoms of travel sickness (sniffing, foaming at the mouth, chomping, retching and vomiting).

We investigate the concentration and short-range order dependence of the zero-temperature resistivity and thermopower for substitutionally disordered alloys from a first-principles approach. The alloy disorder is simulated by calculating the electronic structure of a large supercell (typically 200–250 atoms) with periodic boundary conditions. For the strong-scattering alloys we consider, the electron mean-free path is much less than the supercell dimension, causing artificial effects of periodicity to be negligible. In spite of strong scattering, there is no evidence for localized states near EF. The resistivity and thermopower are averaged over several configurations resulting in statistical error bounds of approximately ±10%. The concentration-dependent resistivity of substitutional V1−xAlx alloys agree well with Korringa-Kohn-Rostoker coherent potential approximation (KKR CPA) calculations. This confirms the accuracy of KKR CPA theory.
